HOW DO I MAKE A CLAIM?

Communication between you, your employer, your doctor and Xchanging is very important for organising and maintaining a return to work. To assist in the management of your work related injury it is important that:

  • you contact your employer and explain the situation to them including how you are, what your doctor has told you about your injury i.e. treatment and restrictions
  • you discuss with your doctor their opinion of your diagnosis, what sort of treatment that you require, why you require the treatment, how often and how long the treatment will be required, any restriction the doctor feels you should follow and any possibility of going back to work and when
  • if you are going to be off work for more than 20 days, then a Return to Work plan needs to be established and a copy sent to your doctor and Xchanging
  • any treatment supports your work related injury and that it is effective in terms of you getting better
  • any change in your treatment is explained to your employer and Xchanging Case Manager, as prior approval is often required especially for things such as surgery
  • as soon as you are able to perform some work, your doctor indicates this on your Certificate of Capacity (for information on how to complete a Certificate of Capacity, click What does a valid Certificate of Capacity look like?)
  • you discuss any difficulties with routine tasks that you performed prior to your injury with your employer and Xchanging Case Manager
  • if you and your doctor are unsure about your capacity to return to work then discuss referral to an approved Occupational Rehabilitation Service with both your employer and Xchanging Case Manager.
